You cannot get blood at Surat Raktadan Kendra without giving properly filled up form signed but the doctor and seal of the hospital. Patient’s two samples along with the requisition form are to be submitted at the receiving counter. If the form is incomplete, samples are not properly labeled or there is any discrepancy between details written in the form and samples, they are not accepted. You will have to go back to get the discrepancies corrected.

Wait till blood group is checked.

You will be given a token and time of issue of blood depending on the availability of particular unit and urgency of your patient.

Do not forget to bring the token while you come to collect blood.

Blood is issued in plastic bags along with a BT set and the cross match form.

Immediately go to the hospital and hand over the blood to the on duty doctor as it must be transfused as early as possible.
