| Hematopoietic Stem Cell Banking: Research and Development |
| Introduction |
|
"Hematopoietic stems cell are multipotent cells, capable of giving rise to multiple hematopoietic lineages to generate different types of blood cells. They are required for the transplantation to following categories of patients.
|

Hematological malignancies likes Leukemia, Hodgkins disease, non-Hodgkins lymphoma, multiple myelonia etc.
|

Hemaglobinopathies like Beta Thalassemia and sickle cell diseases. |
| • |
Non-malignant hematological diseases like congenital immunodeficiency and aplastic anemia. |
| • |
Selected solid tumors like breast cancer and neuroblastoma. |

| Aims and Objectives |
|
This project will be undertaken at Surat Raktadan Kendra and Research Centre (SRKRC) with following aims and objectives:

| Beneficiaries |
|
The main beneficiaries of this project will be the families having a child suffering from beta thalassemia major. The couple can plan a normal/beta thalassemia trait pregnancy and the stem cells of umbilical cord blood collected at the time of child birth could be used for the Beta thalassemia major sibling. We can also store the stem cells for transplantation required for various conditions.

Umbilical cord blood will be collected in the labor room of the maternity hospital, but sterile set up is require for processing the cord blood unit.

| b) |
Stem cell laboratory |
| |
A sterile laboratory is required for processing all types of stem cells. The CD34 positive cell count of the collected stem cells determines the success of transplantation. Hence in this laboratory assay will be developed for CD34 cell measurement.

Cell culture laboratory |
| |
This laboratory will be required to expand the CD34 cells by cell culture |

| d) |
HLA laboratory |
| |
Compatibility of HLA A, B, C and DR antigens between the transplantation action recipient and donor is very essential for successful transplantation. Similarly donor should also be tested for HLA antibodies.
Transplantation program would need healthy donors who are already typed for HLA antigens, Their database will be useful for finding unrelated match for the patient requiring bone marrow transplantation. Thus HLA laboratory can also investigate the donors to create HLA databse.

Cryopreservation facilities |
| |
Harvested stem cells need preservation at -196oC is liquid nitrogen. Special infrastructure is necessary keeping liquid nitrogen tanks. Since liquid nitrogen evaporates quickly there is a need to refill the tanks daily. If space available then it is preferably to erect the liquid nitrogen.
This project would need about 10,000 to 15,000 square feet area for different laboratories and cryo-preservation facilities. Additional space would be required on the ground if liquid nitrogen plant is to be established.
